Nothing (2003)

Main characters discuss an event where oral sex was performed on the other by a dog.

Scene of characters sawing each other (cannot be seen but can be heard and is implied). Then a severed head is shown a couple of times but it's not very graphic nor believable.

Given the R rating, there's strong language in this movie. It's not a massive amount however and probably not more than around 20 uses of fuck.
