During the second season, Guts gets violently beaten in a fight with one of Griffith's apostles. We see him spit up blood and hear some of his ribs to have been broken. He later dons the berserker armor to turn the tide of the battle.
In the second season, we see women being raped by trolls in a brief scene, but nudity is not lingered on. In a horrific scene, a woman in pain crawls to two of the protagonists before we see her stomach bulging and her expressions escalate to pained yelling shortly. Also, we see a troll bloodily burst out of her stomach and it is implied she dies then.
Much darker and more violent than the original anime series.

The 2016 anime contains graphic violence throughout. Guts kills several humans and demons using a massive sword called the Dragonslayer. Bodies are graphically cut in half, limbs are severed and people are impaled by Guts several times per episode. Also, the anime features some intense battles.
A demon-possessed little girl kills her elderly father offscreen. She then holds up his bloody, decapitated head proudly. Guts then kills her by slicing her in half.
Guts kills several knights by slicing them apart, he kills about twenty of them before he collapses from his wounds. He is then imprisoned and interrogated, being whipped as it leaves bloody marks across his chest. The scene is short.
Farnese, being a religious knight, practices self-flagellation to purify herself from impure thoughts. She is shown whipping herself in one scene, nothing too bloody though.
A massive dog-like demon is bloodily split in half by Guts. Later on, Guts graphically cuts a possessed horse in half, the corpse is briefly shown.
A few characters are shown being burned at the stake, falsely executed for being heretics. Nothing especially violent is shown or heard, but charred skeletons are shown afterwards in brief detail.
Mozgus' executioners torture and kill several innocent people and rebels throughout, and often in graphic and cruel ways. They slice people in half, pluck out their eyes, burn people alive and pierce them with spikes. One partly offscreen scene shows their use of the breaking wheel: a method where they break the limbs of a prisoner and bend the broken limbs into the spokes of a wagon wheel. The prisoners are then hoisted up and tied to the wheels, left to be supposedly eaten by wild birds.
Mozgus kills a man by hitting his head with a Bible, some blood.
Mozgus and his men torture a woman via burning of the feet, her pained shouting is heard shortly. Several bloody people are shown being tortured in the background, their pained expressions are shown but the sounds are subdued.
In a pagan cult, we briefly see a container of soup that is implied to be made of human body parts and remains.
Guts fights a giant, goat-headed demon. The fight is bloody, as the demon is shot with multiple arrows and part of its face is blown off by an explosive. Guts finishes the demon off by decapitating it.
Nina is captured and is lead through the torture chamber, where she sees several people being tortured in disturbing and violent ways. We later see her walking back with one of her fingernails torn off.
The spirits of everyone tortured and killed within the Tower of Conviction manifest as a massive wave of black matter, sweeping throughout the tower and killing the guards by dissolving them offscreen.
Mozgus' torturers are individually blown-apart or bloodily cut down by Guts and his allies. One is hit with several bombs before being stabbed through by Guts. One has his throat slit open with blood visible. One has his leg cut off before being shot in the chest with a canon.
Nosferatu Zodd appears in his demon form, slaughtering several men. The bloody aftermath is shown shortly.
